1. Information Annex № 2 to art. 6 of the Ordinance on the conditions and procedures for assessing the impact on the environment, adopted by Decree № 59 of March 7, 2003, Amb. amend. and suppl., SG. 94 of 30.11.2012
2. Documents proving the sponsor of the provision of public access to information under Annex № 2 to art. 6 para. 1 and provide a copy of the paper and electronic copies of relevant / relevant municipality / municipalities, districts / regions and the municipality or municipalities, in accordance with art. 6 para. 9 of the Regulation adopted by Decree № 59/2003, the last recorded. amend. and suppl., SG. 94 of 2012
3. Information about the public interest, if any, was shown in Art. 6 para. 9 of the Regulation adopted by Decree № 59/2003, the last recorded. amend. and suppl., SG. 94 of 2012, including received written comments from interested parties, complaints, objections or proposals, minutes of meetings, other than those provided by the sponsor public access to information under Annex № 2 to art. 6.
4. Evaluation of art. 99a of the EPA (the cases of art. 118, para 2 of EPA) - one paper copy and two copies on electronic media;
5. Document for paid fee.
